Wish for $150 I would get more than 5 format

Cognito is the best online form software I have found so far! I love the professional look of your forms and how customizable the forms and especially the validation checking can be.

Love this form-building platform. It's easy to use and rather intuitive. It's customization options give us a ton of leeway with just the free version, and we are thinking of upgrading to the next pricing plan. However it'd be great if there was a simple video that showed the difference in the plans, not just a list. Or maybe if the list included live links to what each of those upgrades entailed. As it stands, I have to open two pages: the plan comparison page, and a separate window for the Features info page so it can explain everything. And even that is a bit clunky, because some of the features are bundled together, making searching difficult. I just want to be sure that an upgrade is actually necessary before I do it, and the current set up makes it a bit tough to do so.

My other issue is with printing. I wish I could customize what exactly gets printed on the form's PDF. For example, I tend to write a lot of text on my forms, in order to prompt users or ensure they are answering correctly. This is just for them, not for us internally. So when I print, I don't need to see all that text and prompts that turn a 1-page document a 4-page document. So just like I can create custom views for the entry page, it'd be great to do the same for printing. I wonder if any other users feel the same way or if it's just us....

Your product is affordable and for the first year has met the needs of our organization. We will soon grow out of it because we have more complex needs. However, for the basics, it has been great-easy to use and comprehensive in form building. Mostly, your support as been exceptional.

Very good overall! Biggest improvement is the uploaded image orientation—it's killing us! Also, if I declare a field as a numeric input (example: how many stories tall is your building?), I'd love for the mobile keyboard to default to the numeric entry to avoid input errors and validations.

Very easy to use. Used the free version for a motocross event. Went without a glitch. Better than google forms that I have used for the previous event.

I would like to see the ASA status put at the end of the form. Often the ASA status changes as we go through he health history,

Excellent product. Recommendations: keep Trello more up to date with status. I was following a few features which were critical for us, and they were in the in progress queue for years with no status update. Need more frequent updates. Also, the ability to reference data in other forms. Also the ability to import records. Also, the ability for a table view of all the detail records in a master-detail relationship. Not just a view of those detail records attached to a master record, but a table view of the entire detail table.

Only recommendation I would make would be to make themes available or someway to personalize the forms.

I think the web form product has a lot of potential to make things work smoother at our company. It has been a slow process getting the form we've implemented into use - our problem, nothing on your end. I have plans to implement breakfast and dinner carry-out services based on your product next quarter. I am excited about your form service!

I would love to be able to put a border around "content" fields or shade with a color.
