I have tried your form but found that it was not adequate for my needs so I end up making my own codes that actually works and does what I need it to do. Thank you for your service but in my case, I do not need it.
Now if I would recommend this to a friend? well, that depends on his/her preference if they need a simple form like yours, then yes however if they need a complex form as mine, then no.

I love Cognito forms, I love and how easy it is to create your own form. I love how easy is to include calculation formulas. I don't think it's possible to make it any easier. The only gripe I find a bit annoying is the instructions bar/menu that pops up when you point your mouse to a field or something on the form. The reason I dislike this menu bar is that it covers up what you are reading or writing about.

Cognito forms was exactly what we were looking for. As a Canadian immigration practice when a client retains us to prepare an application we need to gather a lot of information such as education, work, address and travel history dating back many years. We wanted a form builder which had a table function and the ability to add as many rows as a client needed as some need to add many entries to their travel history. Word or PDF documents can look a little messy sometimes and often we cannot enter more rows. Therefore Cognito forms solved this problem for us. In addition to that the forms we send out now are aesthetically beautiful, divided into manageable sections, enable a client to save their entries and return as needed and look very professional.
Also the staff are very helpful and responsive. Compared to other form builders I have tried I can safely say that Cognito Forms is the very best.
